What are the requirements for Admission?
1. Two (2) accomplished student`s application forms. (click here to download)
2. Photocopy of Philippine Statistics Authority (PSA) live birth certificate.
3. Photocopy of baptismal certificate.
4. Photocopy of report card of latest grading period.
5. Two (2) pcs. 1×1 ID picture.
6. Two (2) accomplished recommendation forms to be filled out by the Teacher Adviser and Guidance Counselor (for high school students only). (click here to download)
7. Certificate of good moral character (for grades 4-6 only).
For Foreign Applicant
8. Passport, study permit or SRRV (original and photocopy)
9. Transcript of records from the previous school abroad.
